Course Learning Outcomes

Upon successful completion of this course, the student will be able to:

1. Create a comprehensive culturally appropriate plan of care with clients experiencing chronic physical and mental health conditions and disabilities affecting functional status and including issues impacting family relationships

2. Apply evidence-based nursing interventions in support of clients experiencing chronic conditions throughout the lifespan, including:

-palliative care

-end of life care

3. Implement measures to enhance quality of life for clients with chronic physical and mental health conditions and disabilities affecting functional status in the plan of care

4. Analyze appropriate community resources to provide support for clients, families, and caregivers based on:

-health system barriers

-finance

-chronic illness management

5. Examine potential legal and ethical issues related to client decision-making and informed consent in chronic care settings

6. Analyze data, resources, and available technologies to minimize risk of harm to self, clients, and others
